Job Description:
A company is looking for a Nurse Practitioner, Comprehensive
Health Assessment.
Key Responsibilities
Conduct comprehensive health assessments and develop care plans for
members using a telemedicine platform
Evaluate risk scores, assess gaps in care, and collaborate with
care teams to ensure members' needs are met
Participate in emergency coverage as needed and adhere to
documentation practices to maintain high-quality care
Required Qualifications, Training, and Education
At least 3 years of Nurse Practitioner experience, including 2
years in adult oncology and 1 year in telehealth
Possession of a compact RN license and an unrestricted Advanced
Practice license
Experience in developing evidence-based, member-centered care
plans
Comfort with using technology, including video chatting and
electronic health records
Ability to manage multiple tasks and work under pressure while
maintaining organization
